OMIC (“we” or “us”) is committed to keeping your personally identifiable information private. The phrases “personally identifiable information” or “personal information” mean any information by which you can be identified, such as your name, address, or telephone number. This privacy statement describes:
- what categories of personal information OMIC collects;
- the purposes for collecting the information;
- who we might disclose the information to;
- choices you can make about how we use and disclose your information;
- how to make changes to this collected data; and
- how we use your email address.
By accessing OMIC’s website and using our services, you agree to the terms and conditions of this Privacy Statement. Whenever you submit personal information to OMIC, you consent to the collection, use, and disclosure of that information as described in this Privacy Statement.
Information We Collect and How We Use It
Automatically Collected Information, How We Use It, and Who We Share It With
As is true of most websites, each time you visit OMIC’s website, we gather such information as your domain name, your internet protocol (IP) address, and your browser type, and store it in log files. Our server software also records how often you access our web pages. We use this information internally to show aggregate activity and determine potential trends in website usage. OMIC may share this information with law enforcement or forensic consultants in the case of a security breach. Your IP address and page hits can be tracked by Google Analytics in order to provide us with website usage information.
In addition, in order to reproduce your information on the Electronic Applications (eApp) Forms and thus save you the trouble of re-entering it upon a subsequent access of the page, we do link information that we store in a cookie to personally identifiable information that you submit. You can remove the cookie by following the directions provided in your Internet browser’s “help” file.
We may track your browsing history, browser settings, IP address, data entered into our online forms, as well as any data collected via Google Analytics. If you choose to turn on any “Do Not Track” settings on your browser, or choose not to accept tracking cookies, we will exclude Google Analytics tracking during your visit. If you choose to accept tracking, Google Analytics will load; however, we will anonymize your IP address before sending any information to Google. On OMIC’s account creation page, you may be tracked via Google’s reCaptcha technology in order to determine that you are not a bot. Google’s reCaptcha technology may track you regardless of your “Do Not Track” settings or cookie acceptance. Other than for Google Analytics and Google reCaptcha, OMIC will not share any of your automatically collected information with third parties.

Information Not Automatically Collected
The following are the categories of information we may collect about you:

- Your employment or professional information (e.g., your American Academy of Ophthalmology membership status and AAO ID, your medical education, your subspecialty and practice details, your licensure, and your employment history)
- Your insurance policy information (e.g., your policy number and expiration date, your prior policy information and claims history)
- Your financial information (e.g., billing or credit card information)
- Your health information (e.g., medical conditions affecting your practice)
- Your civil or criminal legal history (e.g., prior lawsuits or criminal convictions)
Sources for This Information
The following are the non-public sources of the information we may collect about you:
- You are the primary source for information we collect about you.
- OMIC may also receive information about you from your insurance broker or employer who is obtaining insurance on your behalf.
- OMIC may inquire with ophthalmology practice networks (e.g., ProVision, Solo Eye Network) regarding your membership in order to determine your qualification for insurance and premium rating.
- With your authorization, we may also collect information about you from other insurance companies, hospitals, teaching institutions, professional associations, licensing agencies, and other persons who may have information pertaining to your application and qualifications for insurance, or medical professional liability claims under review.
How We Use This Information
The following are the purposes we may use your information for:
- We may use your personal information to respond to your requests and provide you service. This information allows us to verify your identity and eligibility for insurance with OMIC, underwrite your application, determine premium, process payments, and perform services under your insurance policy with OMIC. We may also use this information to update OMIC’s records.
- We may use your personal information to communicate with you. This might be in the form of sending you information regarding our services; updates to our coverage terms and conditions; changes to our Bylaws; risk management bulletins; and other important policy communications.
- We may use your personal information for marketing purposes. We may notify you about OMIC-affiliated insurance programs, such as business and life and health products.
- We may use your personal information to give you access to our website.
- We may use your personal information for internal business purposes. This may include data analysis, audits, developing new insurance programs, and improving our services.
- We may use your personal information for legal and regulatory purposes. For example, we may use the information to comply with federal, state, or local laws; law enforcement; a subpoena or court order; a governmental investigation or our tax reporting; fraud prevention; and Office of Foreign Assets Control obligations; or to defend ourselves in legal actions.
- We reserve the right to disclose your personal information when we believe that disclosure is necessary to protect our rights, operations, property, or employees.
Who We Share This Information With
- We may share this information with third parties who provide services to OMIC, for example, payment processors, IT vendors, and administrative service providers. We grant such service providers access to your personal information only as needed to perform their services. We require them to protect the confidentiality and security of this shared information.
- We may share this information with third parties who provide services on OMIC’s behalf or who are OMIC’s commercial partners, such as, claims administrators, appointed legal counsel, reinsurers, and affiliated insurance brokers and providers. For example, if you request information about a life or health insurance product, we will share your contact information with the program manager, which coordinates the Academy/OMIC life and health insurance program. Commercial partners may contact you regarding their products and services. However, OMIC does not disclose your personal information to unaffiliated third parties for their own marketing purposes.
- We may share this information with law enforcement, the government, or the courts, as described above.
- We may share this information with others at your request or with your prior authorization.
OMIC never sells your personal information for marketing or any other purpose.
We do not give your personal information to third parties for any reason not disclosed in this statement. We will never use or share your information in ways not described in this Privacy Statement without also providing you an opportunity to opt-out or otherwise prohibit such use.
Please be aware that anyone you provide with your website login information will have access to your account and therefore some of your personally identifiable information.
How We Use Your Email Address
If your email address was voluntarily submitted to OMIC, in addition to using it to respond to your online requests, we may also use that email address to communicate with you about your application or policy or our services. We may also send you alerts, updates, and announcements about your insurance with OMIC, including, but not limited to, underwriting and risk management information. OMIC shares your email address with third parties only as explained above.

If you do not wish to receive emails from OMIC, you may unsubscribe by doing one of the following:
- follow the instructions for unsubscribing on any alert, update, or announcement email from OMIC; or
- contact OMIC as indicated below.
Note that you may have more than one email on file with OMIC. Please contact OMIC if you would like to be contacted at different emails for different types of communications, unsubscribe from some but not all types of communications, or otherwise clarify your email preferences.

How We Protect the Information You Give Us
We care about protecting your personal information. We use generally accepted industry standards to protect your personal information. For information collected on our website, we use a popular and reputable methodology to securely and reliably encrypt all information you provide to us. This makes it exceedingly difficult for unauthorized intruders to read any transmitted data. However, no method of transmission over the Internet or method of storage is 100% secure. Therefore, while we strive to protect your personally identifiable information, we cannot guarantee its absolute security.

OMIC’s website includes a number of hyperlinks to third party websites. They are provided as a service to our users. We do not derive income from clicks. Likewise, other websites may contain hyperlinks to OMIC’s website. We do not pay for such links. The privacy practices described in this Privacy Statement are OMIC’s only. We are not responsible for the privacy practices of any companies whose websites are linked to or from the OMIC website. We reserve the right to modify this privacy statement at any time, so please review it frequently. If we make changes to our privacy policies or practices, we will amend the Privacy Statement to reflect those changes and post the revised version on our website.
Children’s Privacy
OMIC takes children’s privacy seriously. Our website is not intended for children under the age of 13. If you are under the age of 13, please do not use our website.
OMIC does not seek to collect personal information from minors on this website. If we become aware that a person submitting personal information to us though our website is a child under the age of 13, we delete the information as soon as we discover it and do not use it for any purpose. If you believe OMIC possesses such information, please contact us (see below).
